Iron Man takes a real beaten from Madden Mallen who decides to leave the area (running at about 300 miles an hour!) when police helicopters arrive. 

Asked to be taken to Futurepharm, Tony Stark reveals Maya Hasen his real identity and convinces her to get the last live dose of Extremis. Tony wants to become a biological combat machine like his enemy, but also allow his brain to control the armor to be able to operate it faster; Extremis could do that... and also hopefully, save his life because Tony's dying from internal wounds.

After discovering that Mallen is going to Washington DC, Tony updates Extremis and gets a dose. Tony pukes blood. He's not breathing.

Invincible Iron Man (2005 series) #4 Review by (May 21, 2013)
The 1st 3 issues came out monthly, starting immediately after then end of the previous series. But then it took 7 months to get this issue out, and the whole 6-issue arc spanned nearly 1½ years. No wonder Marvel got fed up waiting and started a mini-series IM: Inevitable in the interim (not counting IM: House of M). Maya Hansen is surprised to find that Tony Stark is Iron Man. But it is even more surprising that she doesn't say "still Iron Man", given that in v3#35 he revealed his identity, and then only recently in v3#89 claimed to pass on the armour to a new bodyguard. Mallen meets a teen girl who appears to share his hatred of the powers-that-be. But when it turns out he doesn't share his white-supremacist outlook and his belief in taking America back to what he sees as its founding Libertarianist ideas, he kills her. The Extremis process can be tailored to make whatever changes in your body that you want. The Iron Man armour had become cumbersome over the years, hence needing a crate to transport it. Tony has devised a new version flexible enough to be stored in a briefcase, as it used to be long ago. But it couldn't accommodate all the control systems needed now. Stark gets Maya to set Extremis to build the control system into his body.
